Activity Proposal Form -- This is an on-line form to be completed and submitted to IDOC sites to launch pickleball in their facility. Some sites request the submission; some do not.  Best to wait until after you connect with the Volunteer Services Coordinator at a site and see if they want pickleball and want the form completed. 

Warden Proposal Letter -- Letter to submit to warden to propose pickleball at his/her facility. Not used often because it's best to contact the Volunteer Services Coordinators for each site first.
